The Twenty Club was founded in 1908 as a private Liverpool based dining club for registered medical practitioners, and is one of the oldest medical dining societies in Britain. There are twenty Full Members, approximately one being elected each year and progressing to become President after 20 years. After a year in office, the President resigns and becomes a Senior Member, making way for a new Full Member to be elected.

The Club originally met in the houses of individual members by rotation but subsequently moved to the University Club. In recent years, meetings have been held in the Liverpool Medical Institution. The Club is grateful for the hospitality of the Institution, but there is no formal link between the Club and the Institution or with any other medical, religious or other form of society. A similar club, the Innominate Club, has met regularly in Liverpool since 1930, and joins with the Twenty Club to hold a Burns Night supper in late January.
